WAIVER OF LIABILITY

Please carefully read the following Waiver of Liability terms and conditions.
I hereby release and hold harmless The Club, its officers, employees, agents and any other affiliated companies from any liability with respect to injury of any and all nature to me or my personal property arising out of or connected with my exercise participation or equipment. By signing this form and providing my contact information, I hereby consent to Young's Point Fitness and Lakefield Fitness contacting my telephone and email regardless of whether I am on any Do Not Call registries. I Agree to give at least 5 days notice to cancel any pre authorized payments. I agree to spray and wipe down every surface I touch with supplies provided. I agree to not allow anyone entrance to the club, all members must enter with their own key fob. I agree to wear only indoor footwear in the club.
